What is Support Coordination?

Support Coordination can assist you to understand and implement your NDIS plan. We help find the right service providers to meet your needs and coordinate supports including; mainstream, informal, community and funded supports.

There are three levels of Support Coordination:

Support Connection


Short-term assistance to implement NDIS plans, including identifying service providers and negotiating Service Agreements. This service is provided by a Local Area Coordinator (LAC).

Support Coordination


Support to build your capacity to navigate services and supports you need from a range of providers.

Specialist Support Coordination


Specialist level of Support Coordination by a qualified Allied Health or Nursing Professional to assist during times of crisis, complex circumstances, or through a transition period.

How can we help?

We can help you to navigate the NDIS through the different stages of your NDIS journey. We can provide support to identify the services and service providers that best meet your individual needs. We will also coordinate service providers and supports, or build your capacity to coordinate your own supports.

Eunoia Lane will obtain quotes for services and set up Service Agreements with providers – sourcing suitable accommodation and respite options. We can link you into a job, or other community activities and coordinate relevant Allied and Specialist assessments and reports.

We can respond to a crisis; or identify solutions for managing risky situations. Eunoia Lane will plan and explore options for times of key life transitions, such as leaving school or moving home. We will monitor your NDIS plan outcomes and expenditure, and report back to the NDIA, and re-develop individual goals prior to NDIS plan reviews.
